Transesophageal atrial pacing
combined with flecainide in treatment of atrial flutter in elderly patients

Abstract

Forty-one patients (pts) admitted to our clinic
because of episode of paroxysmal atrial flutter were treated by transesophageal atrial
pacing (TAP) (overdrive, and bursts 300-1500 imp/min) followed by administration of iv
flecainide (bolus 50 mg, and next infusion up to 2 mg/kg) in persons with induced atrial
fibrillation (AF). In the case of atrial flutter return a second TAP was performed. TAP
alone restored sinus rhythm in 17 pts (43%) and when combined with flecainide in 38 pts
(85%). In 4 and 2 pts AF and no effect respectively were observed.
Conclusion. The results obtained indicate that both TAP performance
and flecainide administration in elderly patients are efficacious and safe methods for
interrupting AFl. A combined method of application of both means considerably increases
the efficacy of treatment.
