A combination of extensive clinical experience with
the implantable cardioverter defibrillator, together with recent prospective studies like
MADIT and AVID, have answered the questions concerning the life-saving efficacy of the
ICD. However, the overall problem of sudden cardiac death has hardly been impacted, since
very few patients survive their initial arrhythmic episode. This realization has given
impetus to a number of new studies on the role of ICDs for primary prevention. Ironically,
the accumulated evidence that the ICD does save lives is causing difficulties in designing
and implementing these studies.
