Background

Syncope often remains unexplained despite a complete diagnostic evaluation, because of its infrequent and unpredictable nature1-3. The conventional investigations include short-term ECG monitoring as ECG, Holter, external loop recorder and provocative tests such as head-up tilt test and electrophysiological study. Short term ECG monitoring has a low sensitivity and the provocative tests have a low or unknown predictive value.
The experience with the implantable loop recorder (ILR) demonstrates its effectiveness in clinical practice. Despite the good diagnostic yield (65-88%) of the Reveal ILR, 14% of the patients with symptoms were unable to correctly activate the device4.
